Terms of Use
Terms of Use
Effective Date: August 27, 2026
Welcome to the website of SW Premier Health, LLC. By accessing or using this website, you agree to these Terms of Use. If you do not agree with these terms, please do not use this website.
1. Website Information
This website provides general information about SW Premier Health, LLC, our healthcare services, telehealth services, scheduling options, membership programs, and other resources.
This website provides information for general informational and educational purposes and is not intended to replace individualized medical evaluation, diagnosis, or treatment.
2. No Provider-Patient Relationship
Use of this website, including viewing its content, submitting a general inquiry, or contacting the Clinic, does not by itself establish a healthcare provider-patient relationship.
A provider-patient relationship is established only after appropriate registration, consent, clinical evaluation, and acceptance for care by SW Premier Health Clinic.
3. Telehealth Services
SW Premier Health, LLC may provide healthcare services through telehealth.
Telehealth services are available only when the treating healthcare professional is legally authorized to provide care to a patient in the state or jurisdiction where the patient is physically located at the time of the visit.
Patients may be required to verify their identity and physical location at the beginning of a telehealth encounter.
Certain medical conditions cannot be safely or appropriately evaluated or treated through telehealth. We may recommend or require in-person evaluation, urgent care, emergency care, laboratory testing, imaging, or referral to another healthcare professional when clinically appropriate.
4. Medical Emergencies
This website and our telehealth services are not intended for medical emergencies.
If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Do not use website forms, email, appointment requests, or patient portal messages for emergency medical concerns.
5. Prescriptions
Scheduling or completing an appointment does not guarantee a prescription.
The treating healthcare professional makes prescription decisions based on clinical judgment, medical history, examination findings, applicable laws, professional standards, and the patient's individual circumstances.
A provider may decline to prescribe a medication or may require additional evaluation, laboratory testing, medical records, or an in-person examination before prescribing.
6. Laboratory Tests, Imaging, and Referrals
A provider may determine that laboratory testing, diagnostic imaging, specialist evaluation, or in-person medical care is necessary.
Patients are responsible for completing recommended testing or referrals and for any associated costs unless otherwise stated.
7. Appointment and Payment Policies
Patients are responsible for providing accurate information when scheduling appointments and completing registration.
Fees, payment requirements, cancellation policies, refund policies, and membership terms may be provided separately during scheduling, registration, or enrollment.
Payment for a service does not guarantee a particular diagnosis, prescription, treatment, test, referral, medical certification, or other requested clinical outcome.
8. Membership Programs
If SW Premier Health, LLC offers membership plans, enrollment in a membership does not constitute health insurance.
The applicable membership agreement or enrollment terms will describe membership benefits, limitations, included services, exclusions, cancellation requirements, and payment terms.
Services outside the scope of a membership may require additional payment.
9. Forms and Medical Documentation
Requests to complete FMLA, disability, employment, school, leave, accommodation, or other medical documentation are subject to clinical review.
Payment for an appointment or paperwork review does not guarantee that a provider will certify a disability, approve leave, recommend restrictions, or complete documentation as requested.
Additional records, examinations, testing, or specialist evaluation may be required.
10. Patient Responsibilities
By using our healthcare services, you agree to provide accurate and complete information regarding your identity, location, medical history, medications, allergies, symptoms, and other information reasonably necessary for your care.
Providing false, misleading, or incomplete information may interfere with our ability to provide safe and appropriate healthcare.
11. Privacy
Your use of this website is also subject to our Privacy Policy.
We will handle Protected Health Information associated with healthcare services according to applicable privacy laws and our Notice of Privacy Practices, when applicable.
Patients should use designated secure communication methods for sensitive medical information.

Our website may contain links to or integrate with third-party services, including scheduling platforms, electronic health record systems, payment processors, laboratories, pharmacies, and other healthcare resources.
We are not responsible for the availability, content, security, or independent privacy practices of third-party websites or services that we do not control.
