Service Description
Schedule this visit to review FMLA or disability paperwork related to a documented medical condition. Please upload all forms and supporting medical records before your appointment. The provider will review your condition, treatment history, job requirements, and functional limitations. Additional records, testing, or an in-person evaluation may be required. Payment does not guarantee form completion, approval, or support for the requested leave or disability. Forms are completed only when medically appropriate and supported by sufficient documentation. Additional fees may apply for multiple, extensive, revised, or expedited forms.
Cancellation Policy
By scheduling an online visit, you consent to receiving care through secure audio and video technology. You must be physically located in a state where the provider is licensed at the time of your appointment. Your identity, location, and consent may be verified before treatment. Telehealth is not appropriate for every medical condition. The provider may recommend laboratory testing, imaging, an in-person examination, emergency care, or evaluation by another healthcare professional when necessary. Online visits are not emergency services and do not provide 24-hour monitoring. Call 911 or go to the nearest emergency department for chest pain, severe difficulty breathing, stroke symptoms, suicidal thoughts, severe bleeding, loss of consciousness, or another possible emergency. Do not use portal messages for emergencies. A visit does not guarantee a diagnosis, prescription, refill, test, referral, or requested treatment. All decisions are based on your medical information, applicable laws, and the provider’s clinical judgment. Controlled substances are not prescribed unless specifically offered and legally permitted by the clinic. Patients must provide complete and accurate information about symptoms, medications, allergies, medical conditions, and relevant vital signs. Patients must obtain in-person or emergency care when advised. Visit fees cover the provider’s evaluation only. Medications, laboratory tests, imaging, specialists, hospitals, pharmacies, and other outside services are not included. Payment does not guarantee that medication will be prescribed. Completed visits are nonrefundable unless required by law. Appointments must be canceled or rescheduled at least [12/24] hours in advance. Late cancellations and missed appointments may result in a $50 fee. Technology interruptions may occur. If a visit cannot be completed, the clinic may reschedule it or recommend another method of care. Threatening, abusive, fraudulent, or inappropriate behavior may result in termination of the visit or patient-provider relationship. By scheduling or attending an appointment, you agree to this policy and the clinic’s Telehealth Consent, Notice of Privacy Practices, Financial Policy, and applicable service policies.
